Tongue-tie is a very common condition affecting around 10% (or more) of the population. The controversial nature of tongue-tie is due to not having an accurate understanding of what it is, how to diagnose it, how it relates to common problems like nursing difficulties and speech issues, and how to treat it. There are also differing opinions about how to treat it by different disciplines from ENTs to pediatricians, dentists, speech therapists and lactation consultants. In this course, we will present the strong evidence supporting treatment of tongue-tie and lip-tie as it relates to breastfeeding, speech, and dental issues. The patient history, proper exam technique, and making a diagnosis will also be discussed in detail. It is critical to have a solid foundation of what a tongue and lip-tie are, how to recognize them, and the research that backs up the treatment before treatment should begin.

CO2 Laser Ablation of the Capillary Hemangioma of the Lip

The closeness between coagulation depth and oral soft-tissue blood capillary diameters makes the 10,600-nm CO2 laser a precise and efficient photo-thermal ablation tool with excellent coagulation efficiency. Hemangiomas always present a challenge for an oral surgeon due to their vascular nature and may pose a serious bleeding risk. Various surgical methods have been used to treat hemangioma of the lip (scalpel, cryosurgery, electrocautery, pulsed dye laser, Nd:YAG laser, diode laser, and CO2 laser). We consider the flexible fiber 10,600-nm CO2 laser a minimally invasive and typically suture-free surgical modality. It is, in many respects, superior to most of the alternative treatment options. Because of the excellent hemostasis it provides and the minimal damage it causes to the surrounding tissues, a CO2 laser is especially well suited for hemangiomas and other vascular lesions.

Hemostasis and Coagulation with Ablative Soft-Tissue Dental Lasers and Hot-Tip Devices

Erbium laser wavelengths circa 3,000 nm are highly energy efficient and spatially accurate for photothermal ablation; their photothermal coagulation depths are significantly shorter than gingival blood vessel diameters. Carbon dioxide (CO2) laser wavelengths circa 10,000 nm are extremely effective and spatially accurate photothermal ablation tools with exemplary coagulation ability as a result of a close match between their photothermal coagulation depths and the diameters of oral soft-tissue blood capillaries. In nonablative applications, coagulation depth can be increased for long pulses with extended heat propagation-driven coagulation depth in excess of the photothermal coagulation depth. Unlike CO2 and erbium lasers, soft-tissue ablative dental diodes are contact thermomechanical cutting devices. Their coagulation depth depends on the degree of the diode’s glass tip charring and can range from sub-millimeter (heat propagation-driven coagulation for significantly charred tips) to multi-millimeter (photothermal radiant coagulation for poorly charred tips).

The Use of a 10,600-nm CO2 Laser Mandibular Vestibular Extension in a Patient With a Chromosomal Disorder

Vestibuloplasty involves a series of surgical procedures designed to restore alveolar ridge height by lowering the muscles attached to the buccal, labial, and lingual aspects of the jaws. The technique is indicated in cases of insufficient vestibular depth that may result from atrophy of the alveolar ridge and/or high attachment of muscle or movable mucosa. This article focuses on a carbon dioxide (CO2) laser vestibular extension procedure performed in a patient with Klinefelter syndrome, which is caused by a chromosomal abnormality. The 10,600-nm CO2 laser is shown to offer several advantages over a conventional scalpel and other laser wavelengths for soft-tissue pre-prosthetic surgery, including vestibular extension.
